National-scale tokenized RWE: obesity in the UK

Challenge

A highly complex CEM project for a Top 5 pharma client, marked by uncertainty over the value proposition, a shifting landscape, unexpected data cuts, varied affiliate needs, a newly forming cross-functional team, and negative NICE draft guidance.

Solution

Our RWE team used tokenized CPRD Aurum data to link patient records from primary care through to hospital and specialist care (HES) and the ONS mortality registry. We built a longitudinal cohort of 2.7 million patients, a 20% sample of 31 million eligible, stratified into five BMI groups and analysed with inverse probability of treatment weighting for fair comparison.

Outcome

The study revealed a clear dose-response between higher BMI and increased complications, mortality and healthcare resource use, while showing weight-loss interventions remained underused. It demonstrated how policy-relevant RWE can be produced at scale to support population-level NHS planning.
